FORMS
• All treasury requests must be accompanied by the correct form/receipts. These forms can be found online on the Cos Cob School PTA website under the tab Treasurer Forms.
PLEASE CONTINUE TO DOWNLOAD ALL TREASURY FORMS FROM THE WEBSITE WITH EACH REQUEST AS THEY MAY BE UPDATED THROUGHOUT THE YEAR
• To expedite a form faster please email a picture of the form to treasurer@coscobpta.org
PURCHASES
• If you are purchasing items for the PTA, please DO NOT pay sales tax, we are a tax exempt organization. Our tax exempt form can be found in the Treasurer’s Mailbox in the school office, or you can email the Presidents or Treasurer to obtain a digital form.
SALES TAX WILL NOT BE REIMBURSED.
• All expenses/requests require VP approval/signature BEFORE being submitted to Treasurer, expenses over $500 require President approval.
